Each health care plan determines if a service is medically necessary for the diagnosis of the treatment of an injury or illness. This determination is based on such standards as:
* The service is appropriate and required.
* The service is safe and effective according to accepted clinical evidence.
* There is no less intensive or more appropriate diagnostic or treatment
alternative. If the plan determines that a service is not medically necessary, you pay the entire cost.
