Evidence Of Insurability (EOI) Defined

A questionnaire about an applicant's medical history that is provided to the appropriate insurance company. The insurance company uses this information to approve or deny the requested coverage.
An EOI is required in the following cases:
* Employee enrolling in UT Select who previously waived UT medical coverage and does not have proof of other coverage.
* Adding dependents to UT Select who were previously eligible for coverage and do not have proof of other group coverage at time of enrollment.
* Adding coverage of four, five, or six times annual salary during the first 31 calendar days of appointment or adding or increasing employee term life coverage after the first 31 calendar days of appointment.
* Adding $15,000 or $40,000 voluntary term life coverage for spouse.
* Adding Short Term Disability, Long Term Disability or Long Term Care coverage after the first 31 calendar days of appointment.
